Also, please use the form below if you are requesting a refund. Make sure you fill out the whole form & leave your first and last name, address, phone number, and order number. And there's a section to upload your evidence of photos or a very short video that can be taken with your phone (under 1 min or 15MB max), CLEARLY showing the damage or defective product. It is your choice of whether you want to send photos or a video or both. But you must send one or the other. If your video or photo is bigger than 15MB, you can email it to us. Refund requests must be approved by us first, by filling out the form below within 15 days after delivery. Please you MUST request a refund & fill out the form below within 15 days of delivery, or you will not be eligible for a full refund. But you will still be eligible for a partial refund after 15 days of delivery. If you need any help, please contact our customer service team in time to be able to fill out this form, before 15 days of delivery, & so they will have time to help you with filling out this form. You can just sign your initials if you want, but your signature is optional, & not required.
